HoLEp / laser prostate enucleation

HoLEP (Holmium Laser Enucleation of the Prostate)

HoLEP is a modern, minimally invasive treatment for benign prostatic hyperplasia (BPH).
Using a holmium laser, the surgeon removes the overgrown prostate tissue through the urethra without any external cuts, which lowers bleeding risk compared with traditional surgery such as
TURP.

How the procedure works

The operation is performed endoscopically via the urethra. The laser precisely enucleates the obstructing adenoma to reopen the urinary channel and relieve symptoms like weak stream, intermittency, straining, and nocturia.

Key advantages

  • Complete removal of obstructing tissue, reducing BPH recurrence risk.
  • Very low bleeding rates; suitable for many patients on anticoagulants.
  • Often same-day or next-day discharge.
  • Rapid symptom improvement within days.
  • Durable long-term outcomes versus conventional surgery.

Who is a good candidate?

Men with large prostates, failure of medications, catheter dependence, or those needing a safe option while on blood thinners are typical candidates for HoLEP.

Pre-op preparation

  • Laboratory tests and urinalysis.
  • Ultrasound to assess prostate size and bladder condition.
  • Medication review, especially blood thinners.

Recovery and aftercare

Adequate hydration is encouraged after surgery. Avoid heavy exertion for about two weeks.
A temporary urinary catheter may be used for a short period. Stream flow usually improves progressively.
